The necessary camp paperwork is found in each camp's newsletter, and within the camp registration form.

When you print the appropriate camp newsletter, the medical and travel forms for each camp will print at the same time.  The newsletter is for you to keep.  The medical and travel forms must be filled out and mailed to the Total Performance Sports Camps office along with a photocopy of your medical insurance card  before May 15th.  Completed forms should be mailed to:
